5.  Yoshiaki 以 電話 or 電郵 Confirm Booking。

 ①填妥表格後，列印並傳真至 2560-2359。

②填妥表格資料，以電郵傳送至下列郵址。

*  Cancellation *
 ˙報名後，恕不能更改日期。如缺席，不可轉讓學席予他人。

已繳學費, 恕不退還。
